Medication error is an event that can harm the patient, and can endanger
the safety of patients carried out by health workers that can actually be prevented.
Among them is the Intensive Care Unit (ICU) which has the potential to cause
medication error events. The purpose of this study is to find out whether or not
there is medication error in ICU patients at Permata Bunda Purwodadi Hospital.
This research is descriptive research with data collection through
observation of prescription studies in ICU patients with purposive sampling
techniques followed by interviews with the head of pharmaceutical installations of
RS Permata Bunda Purwodadi. Data analysis is done by presenting the number of
events in medication error.
The results of the analysis showed that the smallest incidence of
medication error at the prescribing stage of ICU patients of Permata Bunda
Purwodadi Hospital was the largest occurrence of no SIP with 71 events (100%)
followed by no sign of R/ with 67 occurrences (0.94%), unclear drug name with 10
occurrences (0.14%), no concentration/dosage of preparations with 53
occurrences (0.74%), no dosage form with 31 occurrences (0.43 %), no dosing
units with 46 occurrences (0.64%), no rules of use with incidence (0.11%), no route
of drug administration with 36 kejadia (0.50%), and no paraphmist doctors with 5
occurrences (0.07%). Factors that contribute to the incidence of medication error
are system problems in hospitals, professionals (human resources, including
doctors, pharmacists, and nurses), and documentation.
