IMPLEMENTASI KEPATUHAN PELAPORAN TEMUAN MEDICATION ERROR PELAYANAN RESEP PADA IMPLEMENTASI BUDAYA PATIENT SAFETY DI LIMA PUSKESMAS KOTA SEMARANG
Patient safety is a new discipline in health services that prioritizes
reporting, analysis, and prevention of medical errors that often cause Unexpected
Events (KTD) in health services. Prescription screening activities carried out by
pharmacists to prevent medication errors. The purpose of the study was to
determine the implementation of reporting medication error findings in
prescription services in Patient Safety Culture reporting in five Semarang City
Health Centers.
Non-experimental research, observational method with quantitative and
qualitative data. Data retrieval using a survey approach with questionnaire
instruments and structured depth interview sheets using the parameters of
Permenkes 11 of 2017, the population of this study was for the patient safety
team in five health centers in the city of Semarang.
In this research survey, it can be concluded that compliance with
medication error reporting in prescription services in the implementation of patient
safety culture at the Semarang City Health Center has a very good compliance
score - 66.7% - 25.0%. Associated with the results of direct interviews with
Pharmacy Technical Personnel of 5 puskesmas in Semarang, each Puskesmas
has its own method and application to reduce or prevent medication errors in
prescription services.
