Pharmacy service is one of the high-risk areas in supporting the quality of
health services, no exception in the Ministry of Pharmacy in the hospital.
Because every risk that arises will cause harm to both the patient and to the
hospital. This research aims to manage the implementation of risk management
of outpatient prescription services and is expected to minimize risk to the
outpatient prescription service system.
The Failure Mode and Effect Analysis (FMEA) is one of the methods that
can be used to minimize the risks involved in prescription services. The study
took place in March – May 2020, the retrieval of this research is prospective.
Data is obtained through observation, interviews and documentation.
The FMEA method in the study is expected to minimize the risk of
outpatient recipes in the pharmaceutical installation of the HOSPITAL PKU
Muhammadiyah Gombong District of Kebumen. Through the FMEA method it is
known that the highest RPN level is on failure in drug intake of 168, failure to
read the drug name by 144 and data entry failure of 140. In this research was
made a new design in the form of color label implementation based on
therapeutic class, socialization guide to medical committee, socialization of
confirmation of recipe and socialization of the outpatient pharmacy supply. The
result of the evaluation after the new design is reduced RPN value for failure in
drug retrieval to 112, failure to read drug name to 90 and data entry failure to 84.
